Terms & Conditions

Client Agreement 

1. Purpose of this Agreement

i. This agreement outlines the terms and conditions for services provided by Diverse Dietetics  (ABN 56 584 672 176). By booking or attending an appointment, you acknowledge that you have read, understood, and agreed to these terms.

2. Services Covered

i. This agreement applies to all services offered by Diverse Dietetics, including but not limited to:

i. Individual consultations (initial and review, in-person or telehealth)

ii. Home visits and kitchen-based sessions

iii. Nutrition packages (5-, 7-, and 9-week options)

iv. Food diary review services

v. Corporate workshops and presentations

3. Payment and Deposits

i. A $50 deposit is required to secure all initial appointments.

ii. Full payment (or balance remaining) is due by the day before your appointment.

iii. Accepted payment methods and instructions will be provided at the time of booking.

iv. For package services, payment may be made in advance or on a pre-agreed schedule.

4. Cancellations and Refunds

i. A minimum of 24 hours’ noticeis required for all cancellations or rescheduling.

ii. Cancellations made less than 24 hours before a scheduled initial appointment will result in forfeiture of the $50 deposit.

iii. Extenuating circumstances (e.g. illness, emergencies) will be considered on a case-by-case basis.

iv. Refunds for partially completed packages are not guaranteed and will be considered based on individual circumstances.

5. Scope of Practice

i. All services are provided by an Accredited Practising Dietitian (APD) within the recognised scope of dietetic practice in Australia.

ii. Dietitians do not diagnose medical conditions. Clients will be referred to their GP or other qualified professionals where appropriate.

6. Client Responsibilities

i. You agree to:

i. Provide accurate and complete information about your health, lifestyle, medications, and medical history.

ii. Inform the dietitian of any changes in your health status or medications.

iii. Follow safety instructions during home or kitchen-based sessions.

iv. Seek medical advice promptly for any symptoms or conditions beyond the dietitian’s scope.

7. Confidentiality and Privacy

i. Your personal and health information is collected and stored securely in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles (APPs).

ii. Information will only be shared with your consent or when required by law.

iii. You have the right to access your records and request corrections if needed.

8. Telehealth Consultations

i. When attending telehealth appointments, you are responsible for ensuring a private, distraction-free environment and reliable internet connection.

ii. Sessions will not be recorded without explicit consent.

9. Use of Heidi Health Transcription Services

i. Diverse Dietetics uses Heidi Health’s Transcribe & Dictate feature to securely transcribe consultation notes.
This system uses encrypted, Australian-based technology to convert spoken notes into text for record-keeping and continuity of care.

ii. The transcription process occurs within Heidi Health’s secure environment and complies with the Privacy Act 1988 (Cth) and Australian Privacy Principles (APPs).

iii. No identifiable client recordings or transcripts are stored outside of Heidi Health.

iv. Transcribed notes are reviewed and verified by your dietitian before being added to your health record.

v. Transcription data is not used for marketing, training, or shared with unauthorised third parties.

vi. By receiving services from Diverse Dietetics, you consent to the use of Heidi Health’s transcription technology for accurate documentation of your sessions.

10. Intellectual Property

i. All educational materials, meal plans, and resources provided by Diverse Dietetics are for personal use only.

ii. These materials remain the intellectual property of Diverse Dietetics and may not be reproduced, shared, or distributed without written permission.

11. Limitation of Liability

i. Nutrition advice provided is based on the information you share and current evidence-based practice.

ii. Diverse Dietetics accepts no liability for adverse outcomes that result from withholding information, non-adherence to recommendations, or circumstances beyond reasonable control.

iii. No guarantee can be made regarding specific health or weight outcomes.

12. Consumer Rights

i. Nothing in this agreement removes or limits your rights under Australian Consumer Law.

ii. If a service does not meet acceptable quality standards, you are entitled to remedies as provided by law.

13. Record Keeping and Retention

i. Client records are securely stored for a minimum of seven (7) years after the last interaction (or until the client turns 25 years of age for minors), in accordance with professional and legal standards.

14. Complaints or Concerns

i. If you have a concern or complaint, please contact lara@diversedietetics.com.auin the first instance.

ii. If your concern is not resolved, you may contact Dietitians Australia or the Office of the Health Ombudsman (QLD).

15. Governing Law

i. This agreement is governed by the laws of Queensland, Australia, and subject to the jurisdiction of Queensland courts.

16. Agreement and Consent

By submitting the online form above you confirm that:

i. You have read and understood this agreement;

ii. You consent to receive dietetic services under these terms; and

iii. You understand your rights and responsibilities as a client.
